    Please complete your New Patient Information forms online through our confidential HIPAA Compliant Patient Portal System. You'll be directed to the NexHealth patient portal to enter your information and fill out a patient health history questionnaire.

  • What happens during the 3-D Digital Wellness Scan?

    The 3-D Digital Wellness Scan provides a detailed view of your teeth and bite, helping us assess your dental health comprehensively.

  • Will I get dental X-rays during my first visit?

    Yes, a full series of dental X-rays will be taken to evaluate your teeth, gums, and bone structure. If needed, a 3-D CBCT scan of your teeth and jaws will also be performed.

  • What kind of evaluations are done during the first visit?

    During your first visit, we will perform:

    • A cosmetic evaluation and screening of your smile and appearance
    • An evaluation of your gum and bone health
    • An assessment of your teeth for decay and fractures
    • A bite and TMJ evaluation
    • An oral cancer screening

  • How is my privacy protected when filling out forms online?

    Our patient forms are available through a HIPAA Compliant Patient Portal System, ensuring your personal and medical information is secure and confidential.

  • What should I expect from the Quality Time with Dr. Brown and/or Dr. Cawley?
